In short: In a black-box test the inner workings are unknown, only outward behaviour is checked.
In more detail: You provide inputs and compare the outputs with the specification without knowing the code.

The tester behaves like an outsider without insider knowledge. That is realistic but finds fewer holes. Counterpart: white-box testing. Intermediate: grey box. See pentesting.
